Here's what you need to do:
1. Please provide two quality color photos (4x4 up to 4x6 size) with 3/4 front and 3/4 rear views of your vehicle. Head or body shots for other packages may be required. Please note, photos will not be returned.
2. Please provide brief personal information: name, address, city, state, and vehicle model/type/accessories, license plate number, and vehicle identification number (VIN). You may include optional information such as: insurance and medical information, emergency contact, and vehicle color(s), condition, and marking(s).
3. If purchasing the Standard package, no personal headshot or body photos required, just two photos of the vehicle. Price: $5.00 each.
4. If purchasing the Above-standard package, no headshot or body photo required, but does come with a magnetic strip for personal information coding, and one free photo key-chain card. In addition, Accident, medical and personal property forms are included. Price: $14.95 each.
5. If purchasing the Best Buy package, it includes a choice of a headshot or body photo (dressed, please), plus magnetic striping for personal information coding, and one free photo key-chain card. In addition, Accident, medical and personal property forms are included. Price: $19.95 each.
6. If purchasing the Silver package, you receive all three packages with Accident, medical and personal property forms included. Price: $24.95 each.
